Application

(Enter State or Province name in this field for businesses in the U.S. states Alaska, Delaware, Montana, New Hampshire, Oregon or outside of the United States.)

By submitting this application, I HEREBY CERTIFY that:

1. My Company holds a current and valid Seller’s Permit from the state I've indicated and that I am exempt from sales and use tax on the tangible personal property purchased from Meridian Specialty Yarn Group's Mill House yarn shop.

2. I CERTIFY that I am purchasing un-spun fiber or Yarn in skein or cone form and that I am in the business of selling the same type of tangible personal property.

3. I will resell the item(s) described in paragraph 2, which I am purchasing under this resale certificate in the form of tangible personal property in the regular course of my business operations, and I will do so prior to making any use of the item(s) other than for demonstration and display purposes while holding the item(s) for sale in the regular course of my business. I understand that if I use the item(s) purchased under this certificate in any manner other than as just described, I may owe use taxes based on each item’s purchase price or as otherwise provided by the laws of my state.

4. Note to Purchaser: A person may be guilty of a crime under your State Tax code if the purchaser knows at the time of purchase that he or she will not resell the purchased item prior to any use (other than for retention, demonstration, or display purposes while holding it for resale) and he or she must furnish a resale certificate to the seller as evidence of its exemption from sales or use taxes. Additionally, a person misusing a resale certificate for personal gain or to evade the payment of tax may be liable (for each purchase), for the tax that would have been due, plus a penalty.

5. I FURTHER CERTIFY that if any tangible personal property purchased exempt under this resale certificate is withdrawn from stock or otherwise used in a manner not permitted under the resale certificate, that I will report the tax due under my state’s Sales/Use Tax Law and Rules.
